There are plenty of myths about private health insurance — but not all of them are true. Too expensive? Not worth it? Only for high earners? We’re breaking down the most common myths and showing you why ottonova’s private health insurance could be a smart choice for you.
Let’s check the facts! 🚀

No worries, that’s not true! Private health insurance (PKV) is designed with integrated mechanisms to keep premiums affordable as you age:

  • Sustainable premium calculation: ottonova’s tariffs are calculated conservatively with a realistic interest rate of 1.25% or 1.75%, considering the current financial market and BaFin regulations.
  • Retirement savings within your PKV: On average, 50% of your premiums at ottonova are set aside for your future, ensuring financial security in old age.
  • Additional premium relief option (BEK): Reduce your premiums later by contributing a bit more today. With our premium relief option, you can lower your monthly payments by €200 from age 67 onwards—and the best part? Your employer covers half of the costs.
  • Built-in premium reductions: At 60, the statutory 10% surcharge on contributions is dropped. Once you retire, sick pay insurance is no longer required, and you can apply for an 8.1 % subsidy from the statutory pension insurance.
  • Independent of demographic shifts: Unlike public health insurance, PKV members finance their healthcare and retirement provisions themselves, making private insurance unaffected by demographic changes.

By calculating your premium independently of your income, you could save up to 200 € per month compared to public health insurance and at the same time receive a higher range of benefits for your health!

  • Those who earn well share a maximum contribution of around €1,174.16 per month (childless, incl. additional contribution and long-term care insurance) in public health insurance with their employer - or bear the costs alone as a freelancer or self-employed person.
  • In private health insurance, you could save up to € 3,746 per year, because your contribution is calculated independently of your income.  
  • In private health insurance, you can benefit from a much more comprehensive health coverage: depending on the plan, benefits might include dental treatments incl. teeth cleaning, acupuncture, treatments by alternative practitioners and chiropractors, dental prostheses, subsidy for glasses, chief physician treatment, preventive medical checkups without age limits and more.
  • Your contribution depends on your health status at the time of enrollment, the coverage you want, and—most importantly—the age at which you join. The earlier you switch to private health insurance, the lower your monthly premium.

It’s always said that you can never get out of private health insurance, but that’s simply not true!

There are definitely circumstances under which you can switch back to statutory health insurance. This mainly depends on your age, employment status, and/or income.

Over 55:

  • If you’re over 55 years old, switching back to statutory health insurance is almost impossible.

  • However, in a few rare cases, it’s possible if you’re eligible for family insurance.

Under 55:

  • If your salary as an employee falls below the income threshold, you must switch back to statutory health insurance.

  • Self-employed individuals can return to statutory insurance if they start working full-time again and earn less than €73,800.

  • Under certain conditions, you can also be covered through family insurance via your spouse.

That’s not true!

The myth that your private health insurance (PKV) can cancel your contract due to high costs or a sudden illness after you’ve signed up is persistent—but it’s simply not the case.

Here’s the truth: No private health insurance can drop you just because you get sick or have an accident. As long as you provided accurate information on the health questions during the application process, your contract is binding and cannot be canceled by the insurer.

And don’t worry, your individual premium cannot be increased just because you incur higher costs due to a sudden illness.

While premium adjustments do occur (both in private and statutory health insurance) due to rising healthcare costs, they are never applied to individual customers. Premium increases are subject to strict legal requirements and cannot be arbitrary.

What are the main differences between public and private health insurance?

Public health insurance:

  • Basically, anyone can be insured under statutory health insurance (GKV).
  • However, the benefits in statutory health insurance are not guaranteed and can be changed by the legislator at any time.
  • Your premium depends on your salary, meaning the more you earn, the higher your contribution will be (up to a maximum contribution). High earners share this maximum contribution of around €1,174.16 (2025, childless, including additional contribution and long-term care insurance) with their employer—or bear the full cost as self-employed individuals.

Private health insurance:

  • Employees with a salary above the annual income threshold of €73,800, self-employed persons, civil servants and students are eligible for private insurance.
  • In private health insurance, you benefit from comprehensive top-tier coverage
  • Your premium is calculated independently of your salary. What matters are your health status at the time of entry, the level of coverage you want, and most importantly - your entry age. The earlier you switch to PKV, the lower your monthly premiums will be.

The laws in Germany only allow residents with a limited residency permit to sign up for expat plans. Unfortunately, this means we can only offer this special tariff to expats who are here on a permit, and not those who can live in Germany without any type of visa.

As a regulated health insurance, we cannot just increase premiums any time we want. Premiums can increase when actual health costs in general differ more than 5% from what we calculated. This can only happen due to inflation and rising medical costs.

However, this will never happen for single individuals but rather for all insured people at the same time. Specifically, we cannot increase your premiums because you get sick or because you get older. We also cannot terminate your contract for those reasons.
